sort of OT..?

Those of us who are full timers, and have yet to sign up for ACA, how does it handle us not being in the state we are 'domiciled' in if and when we need medical care? Obviously, we don't have a standard doctor available, nor a hospital, Urgent Care, etc. as we move around.
 I'm trying to see what I have to do to get registered and the website in SD isn't very accessible...anyone else got thru this already?
