Hi Skysoldier. You didn't specify which vent, but as far as I know the  knob or 'winder' handle has a phillips screw in the center to hold the handle  on. The splined shaft is only there to keep the shaft from turning in the  handle. The screw to keep the handle in place. I'm going to guess an 8-32 x 1/2"  flat head phillips. RonB

I am going to complicate things.
 Between the fixture and the knob is a hollow shaft that fits over the splined end of the lift mechanism and has a receiver that fits on the handle.  The screw passes through this shaft to connect to the drive mechanism.  When this happened to me, I found the screw but could not find that shaft.  To make repairs I had to get a new shaft.  Two months later, the old shaft revealed itself, having vibrated out from wherever it was hiding.
 To reattach the handle, you need both.  If the handle can be pushed onto the spline end without bottoming out, the shaft is still there.  If the handle bottoms out against the frame, you are missing the shaft.
 Once you have all the parts - screw, shaft and handle - I suggest that you use a bit of Locktite on the threads when you re-assemble.
