Has anyone had the factory receiver hitch modified to increase straight pull capacity above the factory rated 4,000 pounds?

Search the archives.  I believe others have said the frame extension, not the hitch, needs to be upgraded.

We did have the factory receiver hitch replaced with a beefed-up version, with the frame mounted extension, but the reason was that the original LD receiver hitch was out-of-square.

If you are doing it to tow something that weighs over 4,000 lbs, though, remember that your gross combination weight rating (GCWR)of 20,000 lbs, at least on the current 14,050 lb. Ford chassis, is the MAXIMUM weight your entire Lazy Daze, towed vehicle, towing equipment, supplemental braking system, passengers in the LD, personal effects in the LD and toad, etc. is legally allowed to weigh.  There is no fudging on the GCWR.

I would not want to get too close to that limit.

 Further, the Ford manual says that the towed object should provide its own braking function.  The E-450's brakes are apparently not designed to to deal with more than its 14,050 pound weight.
